Booking Information

Choose the appropriate appointment type below.  We offer both in person appoinments at our Bristol, TN location as well as virtual telehealth visits for Tennessee or Virginia residents, 

Upon booking you will receive an email with intake forms that can be filled out electronically.   You will be asked to provide payment information, however you will not be billed until approximately 24 hours prior to your appointment.  Please refer to the cancellation policy which you will receive in your email for information on rescheduling if needed.

Please note that due to rising costs of processing fees, each appointment is charged a 2.5% service charge for processing fee.
